PARENT FORMS
OPAA PLAYER, PARENT, and MANAGER/COACH CODES OF CONDUCT
The OPAA Board of Directors has approved the adoption of Codes of Conduct for Players,
Parents, and volunteer Coaches and Managers starting with the Spring 2008 season. While
these codes are conduct are straightforward and for the most part common sense, they are
in direct response to incidents at the park which have highlighted the need to identify
unacceptable behavior in clear and unambiguous terms.
CITY OF JOHNS CREEK/OPAA CHILD SAFETY RULES
Our paramount concern is the safety and security of the children in our program. On any given
Saturday during the season, we will have over 1,000 children ranging in age from 4 to 18 years
old at our park. Please take a moment to download and read the following Child Safety Rules. We ask all
volunteers and parents to help us implement and follow these Child Safety Rules in an effort
to make Ocee Park a safe environment for all of our children.
Ocee Park Medical Waiver form
Every season (Spring or Fall), parents must download a new medical
waiver form, fill it out and give it to their managers.
The form asks for important medical information – such as family doctor
and medical history – that Ocee Park needs to protect your child's health.
More importantly, this form provides permission for Ocee Park league
officials to obtain emergency care for your child in the unlikely
event of an accident or injury when the child's parent or guardian
is not at the park.
The form is available in both Word document or PDF format. Also
available below is the official Babe Ruth League tournament medical
waiver form (a 4-part form). Please download and complete this form
for your team manager if your child is selected to a summer
tournament team since this multi-part form must be carried as part
of the team credentials.
Buddy Ball forms
The following are Buddy Ball division player documents.
The medical form is identical to the one used
for baseball/softball but is provided here for convenience. The
waiver form is unique to the Buddy Ball division.
TEAM PARENT FORMS
The following documents are for the Spring
2008 season. The Team Parent FAQ with general information for Team
Parents was updated December 2007. The trophy form to be used for
specific leagues and teams in the Spring 2008 season has also been updated. Note that
only teams in the Tee League, 7-Year Old and 8-Year Old Farm and A Leagues (baseball),
and the Farm League 6-8 (softball) receive trophies and therefore need to fill
out the team trophy form.

OPAA/CITY of JOHNS CREEK VOLUNTEER FORM PACKET
All volunteers associated with the Ocee Park program are required by OPAA
and the City of Johns Creek to complete 3 volunteer forms: a background check authorization form,
a City of Johns Creek volunteer form, and an OPAA volunteer form. Together these forms must be
completed and returned along with a copy of your driver's license. Please download the
forms below (all contained in a single PDF file), complete them, attach a photocopy of your
driver's license, and return them
to mailbox at Ocee Park (on the concession stand wall) or mail them to the address as
identified on page 1 of the download packet.
MANAGER/COACH/TEAM PARENT VOLUNTEER FORMS
To apply for a manager position with the
Ocee Park Athletic Association for the 2008 Spring or Fall season, please
download the following coaching application, complete it, and return it
to the OPAA mailbox at the park or email them to the appropriate
league commissioner. Please address any questions about managing or
coaching to the appropriate league commissioner or to the VP
Baseball Operations or VP Softball Operations as appropriate.
E-mail the completed forms to the appropriate
commissioner for your league (see table below).
LEAGUE COMMISSIONERS for SPRING 2008 season
Each commissioner recommends a roster of
prospective managers for his/her league to the OPAA Board of Directors,
which makes the final appointments.
Volunteer
Forms
All managers, coaches, team parents,
concession workers and umpires need to fill out the volunteer forms below and return
them to league officials. Applications must be reviewed
and approved prior to any volunteer assuming their role for the current season. Note that
the Volunteer Application is a 3-part form: one is a City of Johns Creek background check
authorization form, one is a City
of Johns Creek volunteer application and the other is an OPAA
specific volunteer form. All of these forms must be completed and then accompanied by a
copy of your driver's license.
